We are able to offer, in conjunction with Cheshire West PCT, a wide range of community and practice based services to our patients and these comprise:

Health Care Assistant
Wendy Hornby
The health care assistant may be consulted for the following: blood tests, dressings, removal of stitches and blood pressure monitoring.

District Nurses
These nurses provide home nursing care to patients who are unable to come to the surgery through ill health. Arrangements can be made by the surgery for a district nurse visit. The District Nursing team can be contacted on 01244 312723

Health Visitor
Yvonne Fryer-Taylor
Our health visitor can give advice and help on health care, particularly for expectant mothers, babies, children, the elderly and disabled. She also offers general health promotion advice and runs ‘stop smoking’ clinics.

A Baby Clinic is held each Monday between 2pm and 4pm (appointment not necessary for the baby clinic).

Appointments can be made with the Health Visitor by contacting her on 01244 312679.

Counsellor
The Counsellor is specially trained to help those patients and their relatives who are under stress. Referral when necessary is made by your GP.

Dietician
The dietician can advise on personal weight reduction and diet or advise on any specific aspect of your diet. Referral when necessary is made by your GP.

Midwife and Maternity Medical Services
The community midwife shares your antenatal care with the doctor. The midwives can be contacted on 365166 or a message can be left at the surgery.

Phlebotomist
We have two specially trained phlebotomists who hold a clinic each week at Heath Lane Medical Centre to take blood. Appointments can be booked on 08444 773304.
